Insurance Information
An updated list of health plans I currently accept is posted below.
If you have insurance coverage with a health plan I do not accept, you will be responsible for all charges at the time of services. Please note that some plans will reimburse you a portion of your charges after seeing an out-of-network provider.
It is important to regularly update insurance information at the outset of your care, prior to all visits, at yearly intervals and whenever insurance changes or new cards are issued. If you have not provided me with updated and accurate insurance information, you may have unexpected financial responsibility for office charges.
I make every effort to verify insurance prior to your appointment. If I am unable to verify your insurance eligibility, you will be required to pay for your visit at the time of your appointment. Following the visit, if you provide the correct insurance information to me in a timely manner, I will file a claim on your behalf. I will refund to you any portion that is determined to not be your responsibility.
It is your responsibility to know the details of your insurance coverage by checking with your employer, insurance agent, or by calling the customer service number on the back of your insurance card. It is your responsibility to inform me prior to the visit of any limitations in coverage. The most common instance of this is regarding preventative health visits (annuals / physicals). Often patients have no coverage for preventative health visits, studies, or tests. Other times, patients have coverage for preventative health, but are required to pay for regular office visit charges. Again, it is your responsibility to inform me prior to the visit of any such limitations.
If you have medical insurance, I will submit your insurance forms for payment to your insurance company and will credit such collections to your account. I will bill you for the remaining portion of your balance, if any, once all insurance claims and payments have been received. For your convenience, I can automatically charge outstanding balances upon completion of a Credit Card Authorization Form described here. Any co-payments or co-insurance are due and payable at the time of service. For patients on deductible plans, the full amount of the office visit will be due at time of service if you have not yet met your deductible. Coding for office visits is done via strict adherence to recognized standards. Codes for visit charges cannot be changed after they have been submitted.
For referrals and studies performed outside of the office by other providers, it is your responsibility to know whether preauthorization is required. I will help facilitate getting pre-authorization but it is your responsibility to inform me if such action is needed.
